Hello, I hope this finds you well:

I am writing you to let you know that I will be out of the office from September 29 - October 30th for the birth of my first son, Skyler Michael. While I am away Janice Plummer will be on call for me should you need some assistance with medication or counseling. Janice can be reached at 801-328-4404 ex 2.

I am asking all of my clients to take a look at their medications and check to see that refills are available for this time period. If you need refills please give me a call prior to Sept. 15th so that I can make sure you are covered.
